How do I apply to CIZ for a Dutch Wlz care indication?
What would you like help with?
Detailed explanation, checklist and sources
Apply to CIZ, not CAK, for a Wlz care indication. Prepare evidence of care needs, medical information and the correct signature, then use the official application. Needing extensive help alone does not establish Wlz entitlement. After approval, arrange care with the care office.

1. Check whether Wlz is the right route
This guide covers the ordinary application for an adult in the Netherlands. Wlz generally concerns a lasting need for permanent supervision or care close by around the clock to prevent serious harm, due to a qualifying condition or disability. Being able to summon help and wait safely matters. Use the CIZ Wlz-check; it is not an indication decision. Children, temporary LVG treatment stays, continued psychiatric inpatient treatment and residence or insurance involving another country have special rules; ask CIZ about the appropriate route.
2. Describe care needs and evidence
Gather clinician documents identifying the diagnosis and date, treatment, effect and expected development. Also explain what can go wrong without help during both day and night, when you cannot summon help yourself and what assistance is required. Include an existing care/treatment plan. Follow CIZ's list for condition-specific evidence; diagnosis alone is insufficient. Send medical documents through CIZ's official secure route, not our checklist or public chat.
3. Check who may sign
If you understand the consequences, sign yourself or authorise another person using the CIZ form. Being a relative or care provider does not itself permit signing for you. If you cannot understand the consequences, check legal representation and CIZ's signature check. Supply appropriate evidence, such as a mentorship/curatorship order or applicable power of attorney. If representation is missing, discuss this with CIZ; proof of a pending mentorship or curatorship application may already be relevant. Do not share DigiD login details.
4. Use the official CIZ application
Open the current CIZ application page and choose digital submission or the official postal form. The consulted digital route specifies a laptop/computer, not a phone/tablet; check current instructions. Free client-support assistance is available. Enter contact, GP and insurance details, add the required documents and signature, and keep a copy and acknowledgement. Our preparation submits nothing and awards no care.
5. Follow completeness and assessment
CIZ assesses your situation and may request missing information, call you or arrange an in-person or video home visit; a visit is not always compulsory. CIZ states that the decision arrives within six weeks after the application is complete. The first submission therefore does not necessarily start that period. Answer document requests and ask about completeness and the applicable decision date if delayed. Someone close to you or your care provider may join the discussion.
6. Read the decision and arrange next steps
Check approval/refusal, care profile and start date. CIZ decides access; the care office helps arrange care and CAK calculates any personal contribution. Remaining at home is conditional, not automatic. Coordinate transition with existing providers and funders; do not stop care yourself based on this guide. If refused, municipal or health-insurer support may be possible. If you disagree with the indication decision, promptly follow CIZ's six-week objection route; a new application or complaint does not replace an objection. Our own help request offers legal follow-up, not a medical indication.
Must I move into an institution with a Wlz indication?
No. Wlz care can also be provided at home where safe and feasible in your circumstances. The care office assesses the options. A CIZ indication does not automatically grant every requested home-care arrangement or personal budget.
