How do I ask SVB whether I am insured under the Wlz?
What would you like help with?
Detailed explanation, checklist and sources
An SVB Wlz assessment determines Dutch insurance status and whether Dutch health insurance is required. It is not a CIZ decision granting long-term care. Apply through Mijn SVB or the official form.

1. Which assessment do I need?
SVB assesses insurance under the Long-term Care Act, for example when living or working across borders. SVB states that Wlz-insured people must take out Dutch health insurance. Applying for care due to lasting intensive needs is a separate CIZ procedure.
2. Is an assessment necessary?
For an internship, voluntary work, au-pair work or PhD abroad without a CAK letter, SVB says no assessment is needed; contact your health insurer. For cross-border telework, first check the separate conditions: meeting them means no Wlz assessment is required; otherwise apply.
3. When should I apply?
For planned departure abroad, apply no earlier than eight weeks beforehand. SVB decides only once circumstances actually change, such as the internship or trip starting. If you receive a CAK letter, apply immediately and also monitor that letter's deadlines.
4. Which evidence is required?
Explain countries, residence, work or study and start/end dates. Attach the documents the form requires for your situation, such as evidence of self-employment. Retain copies; missing information can delay assessment.
5. How do I apply for myself or a child?
For yourself, use Mijn SVB or download, print and sign the official form. Use the current postal address on the source page. For a child under 18, SVB specifies online filing with the child's DigiD or the separate child form. From 18, the child applies themselves.
6. What follows submission?
Keep the receipt: onscreen online, or via Berichtenbox for a postal application if you have one. Processing can take eight weeks, longer if information is missing. The decision arrives in Berichtenbox or by post. Filing alone does not establish insurance status; discuss ongoing cover and CAK correspondence with the relevant body.
Is an SVB Wlz assessment the same as a CIZ care decision?
No. SVB determines insurance status. CIZ assesses whether care needs qualify for Wlz care. Neither application replaces the other.
